How Can I Reach A Patient At Cooper University Hospital?

If the patient you are attempting to reach has been admitted to the trauma unit, a hospital representative will not be able to connect you to them. Copies may be obtained by submitting a letter requesting your records. Please indicate the approximate dates you received treatment at the hospital, if you would like all of your records or just select dates as well as the address of the physician that you would like the records forwarded to. Copies of your birth certificate must be obtained through the city or municipality in which you were born. For those born at Cooper University Hospital in Camden, certificates can be obtained through the office of the city clerk.
