The Cooper Advanced Care Center (CACC) provides comprehensive, team-based services for patients with Medicaid and Medicare, with a strong focus on addressing medical needs and social determinants of health (SDOH). Our patient resources include:
Health Coach Services (SDOH Support)
Patients with identified SDOH barriers are connected to dedicated health coaches who provide longitudinal support, including:
• Immediate and long-term food assistance (grocery gift cards, WIC/SNAP applications, food pantry referrals)
• Assistance with applications for housing, utilities, insurance, and other social services
• Support securing durable medical equipment (DME)
• Scheduling and coordination of specialty care appointments
• Appointment accompaniment when requested
• General patient navigation (mail interpretation, DMV appointments, bus passes, life-event support)
• Referrals to community-based organizations for housing, financial assistance, workforce development, and behavioral health services
• Relationship-building to improve adherence to labs, imaging, and specialty follow-up
Transportation Assistance
Transportation to office visits and procedures is supported through Roundtrip rideshares and the Camden Loop.
Diabetes Care and Education
• Multidisciplinary diabetes group visits with Endocrinology, Nutrition, and Diabetes Education, including individual provider time
• Weekly diabetes education classes offered in English and Spanish (provider referral required)
LPN-Led Diabetes and Hypertension Protocol Visits
Licensed Practical Nurses provide protocol-driven visits for uncontrolled hypertension and diabetes in close collaboration with primary care providers.
Integrated Behavioral Health and Psychiatry
• Embedded behavioral health providers see patients in primary care clinics multiple days per week and collaborate closely with PCPs
• On-site Psychiatry services are available Friday afternoons exclusively for UHI patients
Purpose Driven Care Group Visits
Monthly health coach–facilitated group visits focused on reducing social isolation and promoting purpose and engagement. Activities combine peer support, hands-on projects, and community service.
Pharmacy Collaboration
Integrated outpatient pharmacy support provides:
• Medication reconciliation
• Diabetes and inhaler education with titration support
• Medication interaction and side-effect counseling
Specialty Care Access
CACC assists with scheduling appointments across more than 15 specialty services within the Urban Health Institute.
CACC partners with the Camden Coalition through:
• Monthly multidisciplinary care coordination meetings
• Participation in HTN, Housing First, and complex care initiatives
• Staff completion of the Complex Care Certificate Program
• Department-wide training in Trauma-Informed Care
