New Jersey Law and Your Medical Record

A new law in New Jersey requires acute care hospitals and clinical licensed laboratories in the state to provide you with the opportunity to update your medical record with information related to race, ethnicity, sexual orientation, and gender identity. Click here to view the law.

Do you need to provide this information?

No. You may choose not to answer some or all of the questions related to race, ethnicity, sexual orientation, and gender identity. However, Cooper is required by the new law to provide you with the opportunity to provide this information. For each question you will see as an option: Choose not to disclose.

What will Cooper do with this information?

It will become part of your confidential medical record. Your Cooper providers will be able to see it and use this information for your care.

Is it easy to provide this information?

Yes, you can log into your myCooper account here.

Enter your username and password.

  1. Click on the MENU box in the upper left-hand corner.
  2. Scroll down to Account Settings
  3. Click on Personal Information
  4. On your Personal Information page, you will see a box in the middle of the page "Details About Me."
  5. Click on Edit to answer and complete or edit your information.
