Scheduling Process For Mobile Devices

This page includes instructions on how to schedule, check in for, or reschedule your Camden County Vaccination Center appointment on a smartphone using the MyChart mobile app.

Scheduling Vaccine

  • Step 1:  Download the MyChart app from the App Store or Google Play.
  • Step 2:  Create a MyCooper account. If you already have a MyCooper account, log in using your existing username and password.
  • Step 3:  Schedule your vaccine appointment from your MyCooper account.
  • Step 4:  eCheck-In is required to be completed prior to your appointment. This can be done up to 7 days prior to your appointment.
  • Step 5: Arrive at location no earlier than 15 minutes prior to your appointment. 
 

Step-by-Step Instructions

 


Create a MyCooper Account with the MyChart Mobile App

1. Download and install the MyChart app from the App Store or Google Play onto your phone.

2. Search for the word Cooper and tap the Cooper University Hospital logo.

If you already have one or more other healthcare organizations associated with your MyChart app, tap ‘Switch Organizations’ at the top of the screen. Then tap the plus sign next to the words ‘My Organizations’ and search for Cooper.

3. If you already have your own MyCooper account you can skip straight to scheduling your appointment. Otherwise tap Sign Up Now.

4. Tap the link to Sign Up Online:

5. Complete all required fields on the form. When you have finished, be sure to complete the captcha/checkbox at the bottom and then Submit the form.

Please take care to enter your personal information correctly. If you mis-enter any information please do not worry and do not create a second account. You will have the opportunity to make corrections when you arrive for your appointment.

6. Choose your username and password, retype your password and tap Next.

7. Tap Sign In to enable email notifications. You will get an email to remind you of your appointment.

8. Tap Accept for the Terms of Use.

9. If a list of notifications covers part or all of the screen, tap the Bell in the top left hand corner to close the pop up.

Schedule Your Vaccine Appointment using the MyChart Mobile App

1. Open the MyChart app. Make sure the MyCooper logo appears above the sign in fields. Enter your username and password, then tap Log In.

2. Tap the Appointments icon.

3. Tap Schedule an Appointment at the bottom of the screen.

4. Tap on COVID Vaccine.

If you were previously a patient at Cooper Health there may be additional options available, including doctors you’ve seen for care. The COVID vaccine button is located under the words ‘Tell us why you’re coming in’.

5. Answer the required attestation question. Tap Continue.

6. Choose appointment date and time.

7. You may be asked to verify your personal information. Please complete any required fields, and review and make any updates. Once everything is correct, tap This Information Is Correct to continue.

8. You may be asked to verify your insurance. You are not required to enter insurance information for a COVID vaccine given at 200 College Dr., Blackwood, NJ.

If you do not have any insurance information on file, tap This Information Is Correct to continue.

If you have any insurance information on file, first select the button labeled Do not bill insurance. Then tap  This Information Is Correct to continue.

9.Tap Schedule Appointment Now. You must tap the “Schedule Appointment Now” button to create your appointment. Once you have done this a confirmation screen will appear letting you know your appointment has been made.

eCheck-In Your Vaccine Appointment using the MyChart Mobile App

1. You will need to check in online prior to receiving your vaccination. You can check in anytime during the 7 days prior to your appointment.

Open the MyChart app and log in to your MyCooper account. Tap the Appointments icon.

2. Locate your vaccination appointment in your list of upcoming visits. Tap the eCheck-In button.

3. You may be asked to verify your personal information. Please complete any required fields, and review and make any updates. Once everything is correct, tap This Information Is Correct to continue.

4. You will see one or more forms that require your electronic signature. Completing these forms online significantly reduces the time you’ll need to spend at the clinic prior to receiving your vaccination.

To complete a form, tap Review And Sign.

5. Read the form. Scroll down to find the signature field and tap the words Click to Sign. Tap the Continue button at the bottom of the form.

6. Once all forms are signed, tap Submit to finish eCheck-In.

7. Once you’ve completed your eCheck-In, you will get a Barcode. Bring your phone to your vaccine appointment and scan the barcode for touchless appointment arrival.

You can find the barcode by logging in to your My Cooper account, tapping the Appointments icon, and then locating your vaccine appointment in your list of upcoming appointments.

Reschedule Your First Dose Appointment using the MyChart Mobile App

1. You can change the day or time of your vaccine first dose at any point prior to your appointment.

Open the MyChart app and log in to your MyCooper account. Tap the Appointments icon.

2. Locate your vaccination appointment in your list of upcoming visits. Tap to open the details screen for this appointment. Tap Reschedule or Cancel.

3. In the popup, tap Reschedule.

4. Your original appointment information shows at the top of the screen. Scroll down to see appointment times beginning with soonest available.

5. Tap to select a new time for your appointment.

6. Tap Schedule. A confirmation will appear letting you know your appointment has been rescheduled for your chosen day and time.
