Cooper Employee Volunteer Program Application

Fulfilling our mission, To Serve. To Heal. To Educate., Cooper is excited to share volunteer opportunities in the community with a pool of employee volunteers. Through the following application to the Volunteer Program, employees may serve their communities by representing a multidisciplinary range of service lines in an effort to educate, engage, and empower our community.

I acknowledge that I meet the requirements of the Cooper Employee Volunteer Program:

  • Successfully completed my 90-day performance review.
  • Have no written disciplines in my employee file in the past 12 months.
  • Rated as “meets expectations” or above for the most recent performance evaluation.
  • Manager’s permission to participate in this program.
