SAP/DOT Services

Qualified Substance Abuse Professionals

Those individuals who are governed by D.O.T. (Department of Transportation) regulations (CDL, train operators, etc.) who test positive for drugs and/or alcohol must be evaluated by a S.A.P. – Substance Abuse Professional. The following individuals are DOT qualified Substance Abuse Professionals (SAPs):

  • Heather Skarbek, LSW, SAP
  • Ronna Stedman, LSW, SAP

The cost for SAP services is $800.00 payable at initial appointment.

At the time of your initial appointment, you will be required to bring:  

  • a copy of the drug screen results with you.
  • an acceptable forms of payment are CASH, a check from your employer or notarized guarantee of payment for your employer.
  • your insurance card.  

What to expect:

  • The evaluation is approximately 60-90 minutes which includes a SASSI evaluation and a face-to-face assessment by the SAP.
  • Payment will be collected before start of assessment.
  • A copy of your insurance card will be made.
  • SAP clinician will have AA/NA meeting lists for the area, websites which are substance related, a “what to expect” outline, numbers to hotlines, names and phone numbers of local facilities and clinicians, and education about levels of care.
  • SAP will make sure client knows they must call the following day in order to get the level of care recommendation and where they can go for such treatment.
  • Standard of care for a SAP to clear an employee for a return to work / fitness for duty screen will be dependent on the employee’s successful completion of three negative consecutive drug screens (preferably within three days of each other).
  • Be prepared to be out of work for a minimum of 30 days.

In order to be considered for return to work, the individual:

  • Must be compliant with treatment.
  • Must have a face-to-face follow-up evaluation with the SAP.
  • Must have a negative return-to-duty drug screen.
  • Will be required to submit to unannounced follow up drug/alcohol screening as determined by the SAP.
